In August, Wallace is teaming up with Gresham-Barlow School district to make physicals available for students wanting to sign up for sports. The event will take place over 2 weeks in early August. See the flyer for times, dates and locations.
Note: A parent or guardian must complete and sign the history form before the visit. Patients 15 or older may complete it themselves. A phyiscal exam is required to determine sports eligibility. Clearance is not guaranteed until the evaluation is complete.
