Care Beyond The Clinic Walls
This is Wallace’s first winter in our brand-new building. It is an exciting time; the new space comes with many improvements and efficiencies, and represents a big step forward for our clinical care. Yet the most critical factors to good health—nutrition, housing, and more—exist outside the doctor’s office.
That is why we are so proud of the way Wallace treats each patient as a whole person, and addresses these non-medical factors as essential parts of patient health. And we’re humbled by the support of our donors, who make this exceptional care possible.
Patient Navigators
Our Patient Navigators are the team responsible for these interventions. “We make sure our patients have the essentials figured out,” said Patient Navigator Leslie Acosta. “In the past month, we’ve helped patients secure food, shelter, transportation, medications… After two years of COVID, the need is the greatest it’s ever been.”
One such patient is Dorothy. Dorothy had been living in an apartment up several flights of stairs with no elevator. In the last year her mobility deteriorated, and it became harder and harder to navigate her building’s stairwells. With no family or friends to help her, Dorothy knew she needed to move to a new building or risk becoming homebound. This November, Dorothy got on the waiting list for an apartment nearby with an elevator.
When moving day came, Dorothy called Charlie in tears. The movers were unprepared for the size of the move and the number of Dorothy’s belongings. They were about to cancel the job and leave when Charlie sprang into action. She got on the phone with the moving company, explaining Dorothy might lose her spot in line for housing if the move fell through. Meanwhile, she secured emergency patient-support funding to cover the longer move.
“Dorothy was being told she had to choose between a home she couldn’t leave and a home with none of her belongings,” said Charlie. “I knew I had to intervene and get Dorothy moved in right.” That evening, after a successful move, Dorothy told Charlie how grateful she was to have someone she could turn to for help.
